LIFELINE/LINK-UP ASSISTANCE PLANS

The Lifeline program provides assistance to reduce eligible consumers' monthly charges. Under the guidelines of this program in Indiana, $8.25 will be deducted from the monthly local service rate for qualifying low-income customers.

The Link-Up program offers eligible, low-income customers a 50% reduction in service connection charges up to $30. In addition, a deferred payment plan, for charges of up to $200, is available for initiating service, for which eligible customers do not have to pay interest charges for a period not to exceed one year.

To be eligible for participation in these programs, an individual must participate in one of the following: Medicaid, food stamps, Supplemental Security Income (SSI), federal public housing assistance, Low-Income Home Energy Assistance Program (LIHEAP), Temporary Assistance to Needy Families (TANF), or National School Free Lunch Program.

All Qualifying low-income customers will have the following services available to them: single-line local service, touch tone, access to emergency, operator and interexchange services, directory assistance; and toll limitation (the prevention or limitation of long-distance calling).


EXAMPLES OF PROOF TO VERIFY LIFELINE/LINK-UP CREDITS

If you checked any of the following seven (7) programs you will need to show proof such as: a copy of the approved application, a letter of authorization, or a valid card.

Low Income Home Energy Assistance Program (LIHEAP)

Food Stamps

Supplemental Security Income (SSI)

Temporary Assistance to Needy Families (TANF)

National School Free Lunch Program

Federal Public Housing Assistance or Section 8

Medicaid

 

If you qualify because your income is at or below 135% of the federal poverty guidelines you must present:

Prior year’s tax return (state, federal)

Current income statement or paycheck stub (past three (3) months)

Social security statement of benefits (past three (3) months)

Veteran’s Administration statement of benefits (past three (3) months)

Retirement or pension statement of benefits (past three (3) months)

(If you bring in your prior year’s tax return, it will only be read by Bruce Hanway, our General

Manager. He will verify you showed valid proof by signing this form below and returning the tax forms to you.)
